Problem

The starting point for the project was to develop a solution to help the hospital transplant sector. To this end, a lengthy research process began, starting with interviews with doctors and nurses in the field, followed by patients who had already undergone transplants.

The main challenge in the transplant process, identified through interviews with patients and nurses, is the management of test updates and communication with the patient. The nursing team has to frequently check and update the status of the patient's tests, which takes up time that could be used for direct care. In addition, patients often forget to renew their tests, which requires nurses to remind them. Communication is difficult, as many patients are on low incomes, frequently change their contact details or don't have a phone, depending on others for information about their situation in the queue.

Some of the insigths obtained were:

“In Brazil, more than 50,000 people are on the waiting list for an organ transplant”

“The nursing team has to frequently check and update the status of the patient's tests, which takes up time that could be used for direct care.”

“Patients often forget to renew their tests, which requires nurses to remind them.”

We therefore define the problem as:

Process

Moving on to the problem immersion stage, the 5 whys technique was used, a technique used to try to find the root of a problem. The guiding phrase used was:

“Patients miss the opportunity to receive transplant organs”

1 - Why? - There is a lack of access to data and updating this data is tedious

2 - Why? - Nurses are overloaded and can't check

3 - Why? - Because they can't check it, they end up not being able to update it

4 - Why? - There are too many patients and not enough nurses, and the system is not very efficient.

5 - Why? - The lack of dynamism and efficiency of the SNT prevents nurses from doing their best work

For this stage, the knowledge gained from the interviews with nurses was used to produce Personas, empathy maps and a user journey. In this way, empathizing with the pains that the user has present in their current workflow. This was done in order to find a solution that solved the problem and easily entered the user's workflow.

Solution

After all the research and understanding of the problems faced by hospitals and patients, Alerta Saúde was created

Alerta Saúde is a platform for controlling and monitoring data on patients in Brazil's organ transplant queue. It allows patients' data to be entered and updated in a simplified way compared to the current system - the National Transplant System (SNT) - and allows immediate visualization of patients whose tests are overdue. Since these patients run the risk of losing their place in the transplant queue or losing the chance to receive the organ.

Thus, Alerta Saúde reduces the risk of tests not being updated due to human error, allowing more patients to receive an organ via transplant.Alerta Sáude has a few main screens that make it possible to carry out the necessary actions, namely:Home screen - On this screen, the nursing team has access to the total number of patients, as well as the number of patients in the different categories. it is also possible to view the alert center and an initial view of inactive recipients.

Recipients screen - On this screen the nursing team has different filtering options to find each patient and through an intelligent mechanism each recipient who has an exam 15 days overdue receives an automatic alert on all available means of contact.

Patient profile screen - On this screen, the nursing team has access to the patient's data, such as contacts and address, history of consultations, a link to their record and exams. Another important feature is the visualization of the patient's transplant flow, so they can see what stage they are at.
